Patient Confidentiality and Privacy Policy
Our goal is to provide the highest quality care and service
possible. We value the privacy of your health information and
maintaining its confidentiality is one of our highest priorities. In
delivering our services and products, we are committed to the safe and
responsible use of your information.
Providing Service to You
We collect, retain, and use information you provide for purposes of
planning and providing services to you. Your health information may be
shared with others involved in your care. This may include your primary
care provider(s), specialists, and other Maryland Holistics
practitioners who are caring for you. Also, your practitioner may share
certain information when consulting another healthcare provider, to
determine the best type of care for you. Whenever possible, your
personally identifying information will not be used.
Billing
To process your payments with any third-party entity, such as an
insurance company, we may be required to disclose certain information,
such as a diagnosis, procedures performed, and/or treatment progress. A
bill may also be sent to you with this information included.
Quality Assurance
Maryland Holistics may use your information to evaluate the
effectiveness of the care you have received, and to evaluate the
effectiveness of certain types of care in general. Whenever possible,
your personally identifying information will not be used. Our use of
your information for quality assurance purposes, including for risk
management, will enable us to continually improve the effectiveness of
our services and care.
Protecting Your Information Online
All of our operational and data processing systems are maintained in a
secure environment with our third-party processor(s). Our agreements
with all third-party processors is that they follow privacy policies
similar to our own, and keep client information strictly confidential.
Since email is generally not secure, we recommend that you do not
include any sensitive, non-public information in an email to us. We may
send emails to you to facilitate scheduling appointments, to respond to
your questions, and to send you periodic announcements such as our
seasonal newsletter.
Disclosures Required by Law
The following disclosures are required by law and do not require your
consent. We are required to disclose information relating to adverse
effects of supplements and other products to assist the FDA in managing
product recalls and replacements. As required under state or federal
law, your health information may be disclosed to appropriate agencies,
officials, or attorneys: 1) in response to a valid subpoena; 2) in the
event that a staff member or associate of Maryland Holistics has reason
to believe that a client(s) is endangered due to suspected unlawful
conduct of a practitioner or violation of professional clinical
standards; 3) when a client is a suspected victim of abuse, neglect or
domestic violence. We may release information to the extent authorized
by law in matters of worker’s compensation. We are also required by law
to disclose health information to public health and/or legal
authorities to avert a serious threat to public health or safety, to
report communicable disease, injury, or disability, or to comply with
mandated reporting requirements for tracking of birth and morbidity.
Routine Uses and Disclosures
Our policy is to consider the following as routine uses / disclosures
for which we will not request specific authorization. You have the right
to request restrictions on these uses. Using our best judgment, a
family member, close personal friend identified by you, personal
representative, or other persons responsible for your care may be
notified or given information about your care to assist them in
enhancing your wellbeing, or to enable us to confirm your physical
location. Maryland Holistics may send you information about treatment
alternatives and other health-related services or products that you may
find useful. We do not share personal health information about you to
other companies or organizations in connection with any advertising
campaigns.
Maintaining Accurate Information
We strive to maintain accurate information about you. If you ever think
that our records contain inaccurate information about you, or we have
reported inaccurate information about you to an outside agency, please
let us know. We’ll do our best to correct any mistakes immediately, to
the extent that is possible.
